This article aimed to report a case of penile paraffinoma that underwent excision and reconstruction with a scrotal flap technique. In this case, the paraffinoma was caused by an unusual fluid injection, resulting in a unique clinical presentation. We report the case of a 22-year-old young male patient whose penis had been injected with hair oil, by an inexperienced friend, injected twice with a gap of 1 week after the first injection, with a total of 15 cc. The injection took place one year before conducting the surgery. The penile shaft of the patient appeared to be surrounded by rugae and scar tissue without signs of inflammation; the pain was minimal, especially during an erection, and the penile shaft felt harder. The procedure began with the excision of scar tissue, followed by a scrotal flap to close the excision wound on the penile shaft. The histopathological examination results showed a granulomatous inflammation of the penile tissue which was a typical sign of paraffinoma. The evaluation results of 1 month after the procedure showed that the surgical wound was good and there was no infection. There were no disturbances in urination and erection. The patient only complained about the hair on the penile shaft. We can conclude that the scrotal flap is effective for reconstructive procedures in patients with penile paraffinoma, even in cases with a unique clinical picture due to the injection of an unusual substance. © 2024 EDIZIONI MINERVA MEDICA.
